Header/Footer Options for SOAP Note Printouts

It is a requirement for insurance to have name and birthdate on all pages of a printed SOAP note. Can we please have header/footer options available on SOAP notes? It would also be great to select which info is displayed in the header/footer due to ever-changing payor requirements.

    It is a Virginia Medicaid requirement to have a header and footer on each page listing... client name, DOB, and primary and secondary insurance ID.
