Soap Note Header/Footer

I want to be able to add a footer to the SOAP note so that if the SOAP note covers more than one page when printed, there would be identifying information on both pages. We have to print and submit SOAP Notes prior to billing for some of our clients. When the notes are more than one page, it does not have any identifying information on any page other than the first one. I would like it to have the client name, DOB and DOS
